Cut the old cover and foam away from the seatback, and you'll see how the headrest post is retained. There's a small "peg" on the side of the headrest post that must be released past the retainer part in the channel of the seatback.

If you tilt the headrest to one side as you pull out it will release from the seat. I forget which way you have to tilt but you'll figure it out. There's a peg on one side of the bottom of the headrest post that keeps it from pulling right out.
