There are fairly common differences in chromosome characteristics (I've read about this somewhere years ago) that causes some people to have complete resistance to the effects of different kinds of pain killers, such as codeine. That's probably true of Vicodin also, since it is basically just a mixture of acetaminophen (Tylenol) and hydrocodone.

The abuse of substance has to do with way the substance is administered. The painkiller pills are specifically manufactured to provide slow absorption for long term analgesia. People who abuse these drugs have to defeat these slow release mechanisms by "cooking" the pills thereby allowing all the opiates absorbed at once.
