Relax, no need to attack. No matter if it's a CHP officer or a 99 year old woman any death is sad. It is possible the OP was not aware of this. On face value it does seem overkill.

Reach down and pull the mat back. I got all weather mats on top of the carpet and no I have
not noticed once a binding situation in my lexus's.. There has got to be more

This is not limited to any particular make or model. The Lloyds mats I got for my coupe had a large warning not to install them on top of the existing mats, but to replace the originals and use the hooks to secure the new ones. The mats in my 2008 Honda are similarly secured to the floor. Obviously there have been enough problems with mats interfering with proper operation of the pedals to require corrective action. It's known as product evolution.

Most likely more then just the mats. Hope your Lexus is not on the fire hazard recall.

I think it is just that. The mats can move forward and catch the accelerator pedal so when you let up, the pedal is still being pushed forward. Getting thinner mats or getting a shorter gas pedal (further from the floor) would solve the issue.
It was a sad event but if the passenger had time to make a 911 call, the driver had enough time to turn the car off or put it in neutral and stop. The heck with the motor.
This is not so much a Floor Mat issue, but a bad design and implementation of the Accelerator pedal components and placement. Toyota should have known better. It would be nice if they got a serious blow to their perception Armour because of this.
I, personally, have known for a ALOOOOOONG time that Toyotas are about as mediocre as a bowl of stale corn flakes with no sugar or milk.
