For those having the issue we need some basic information like the browser, browser version, screen resolution and also see a screen capture of what you're seeing. The issue is likely a specific setup.

Sounds like you're having the same problem as I. I started a post a couple back and was asked to send a screen capture. I am using an iPhone 5 and the full site. Don't like the mobile site format. Didn't have problems until a few days ago. I am sure the insert and cancel buttons are there. I just can't swipe/scroll far enough down to get to them.

Here you go. These were uploaded with a laptop.

I'm using Google Chrome, on a desk top using Vista; although the issue exists exactly the same using Internet Explorer. I do not know the browser version, and do not know how to obtain; I do not know the screen resolution, and do not know how to obtain; I do not know how to give you a screen capture. If you can walk me through how to do the three things listed, I can give you the information.
Thanks.

I'm thinking the issue is related to screen resolution or possibly an add-on in the browser. The following two are on my system using Firefox and Chrome and the Insert is there.

We'll need more information, such as the screen resolution. In general, this can be found in the Control Panel and Display.

Firefox

Chrome

CTRL and "+" or "-" key works to change display size on PC and I can see the Insert and Cancel buttons. (Firefox 30.0)

Any idea how to change Safari display size on iPhone 5? I can change font size, but that has nothing to do with Safari browser display.

Best I can tell, I have a 1024 x 768 pixels resolution, or low on a sliding scale.....In my screen shot, I can see the "upload more photos from" tab in the lower left hand corner, and also the "Re-order images" tab in the upper right hand corner.....if we connect those XY axis, I should be seeing the cancel insert button, according to the screen shots JT provided, but I don't. Control + or - has no affect on my issue.
Thanks

So no answer to the iPhone 5 issue, huh?

I can confirm that low resolution (1024 x 768) is an issue with the new Attachment system.

I will check with the team about the issue, but 1024 x 768 is a pretty low screen resolution.
