I got mine today.
Be prepared to spend considerable time if you expect to fill it out correctly. You will be asked to expand on every issue that is not 100% to your liking. I expect to have a follow up call.

I filled it out approx. 2 weeks ago, no follow up call and I have many of the problems in issues and resolution thread.

I did the survey a few weeks ago and I did get a call from them to elaborate on my concerns.

I don't recall if I provided my phone number but probably did so we'll see if they call me. I was critical but honest and provided detailed comments when asked.
