The clutch (as some people are calling it) is really a alternator decoupler pulley (ADP) which allows the belt to power the alternator during acceleration and cruising speed, but DECOUPLE or disengage from the alternator during engine deceleration
Looks like this.

It seems the automatic cars have a clutch on the alternator so don't have that issue. Once done properly with Red Loctite, I have not seen anyone have further problems though.

If you buy a new gm alt or have a warranty on your car the new alt has the clutch just had same thing happen after 4k miles nut came off, dealer gave me new alt. I bought a warr w my car for $500 and have saved 4k on broken rear end. Btw when my pulley came loose it wore out the inside of the pulley and wouldn't sit flush on the alt any more.

Most likely you will need a new alternator if the but came off. I put mine back on with lictite and about 100 miles later it sounded like a tin can with stones rattling in it. It worked but bearings I assume were shot. Got a replacement from the dealer.
