By placing my palm on top of the shifter I use three fingers not including the thumb. Try that it might make it a little less painful to move the shifter as the thumb isn't involved shifting into 5th or reverse.

Well I lost my entire nail on my left thumb doing a brake job for my Dad many years ago. Had to boil it out with peroxide every night and it hurt like hell.
Do I hear three, three and a half?? When this happened to me I was almost 22 in 1982, my car then was a 1971 Motion Performance 454. Shifting that beast wasn't fun for a while!!
