The LS1 clutches are sold as kits that contain the flywheel, pressure plate and disc. I suggest the 2002 ZO6 clutch kit. It's a direct bolt on and can handle much more power than the typical LS1 unit.
Part numbers:
2002 ZO6 Clutch Kit: 12570806
2002 Master Cylinder: 12564455
2002 Slave Cylinder: 12562767
The throwout bearing is attached to the end of the slave cylinder so no need to order one, in fact you can't since it's attached to the slave.
