Look at your harmonic balancer, sounds like it is "walking" off the crank.

a couple of things i can think of. look at the pulleys while car is running and see if any wobble and/or check pulleys with belt off for wobble/play. you may not see the wobble with belt pressure. second check the tensioner at top to make sure it is not bent or loose. i also hear that the crank pulley can loosen which causes some to throw belts. good luck.

Your balancer has failed. FIX IT IMMEDIATELY if you don't you will be faced with a much more expensive fix. Ask me how I know! It's the rubber layer between the hub and the outer inertia ring. The outer ring can slide back or forward. If it slides back it will eat through the front of the oil pan and the front engine cover. Very expensive to fix.

The fix requires a few special tools, pulley puller and a tool you can make or buy that puts the new pulley back on. Do not use a stock pulley and you must use a new bolt. Buy an aftermarket pulley from ASP, ATI or Powerbond. You can go underdriven or stock diameter. If you ever wanted to do a cam now is the time as its not that much more work. I did when I did this fix. I also had to replace the front engine cover, oil pump, oil pan and timing chain. so I went the whole enchilada and installed new springs, seals, retainers and cam.

It could cost you alot more if it breaks. Much cheaper to fis it before it fails.
