That label would be model year specific and I would think NOT an issue to be replicated.
These people require you submit a legible digital snapshot for their certification labels etc BUT they might actually do something like this ECM label also. It might be worth a conversation. They don't work "cheap" but they are a GM certified vendor. There are others that offer duplication services. Looking at my '90 original I don't see where there should be any issues.

The second line of information and the bar-code might hold and likely does hold date coded information for the NCRS concerned but I've never seen it decoded.
**** ECS says NO. Since I had a snapshot I just sent the request.

This is from my '90 #653 and I believe it might not be that difficult to replicate:
The font is pretty straight forward and there's certainly similar quality label blanks available BUT how to do the bar code? There would be a total of three different service # LABELS, '90 & '91, '92 & '93 AND '94 & '95. The 4 letter code would change for most every year but it would be certainly easy enough to compile the information. Line 3 is the assembled production ECM number. That along with the 4 letter code is all that would change on each MY presentation.
Just a quick check looked like the bar code might not be that difficult but would require a purchase of a software program. If a label were done on quality material an acetate/vinyl adhesive cover could be applied over the label or maybe the label just be cleared.
