Default gmos04 issue

Having an issue with GMOS04 on a C6 and wondering if anyone else has ran into this

no chimes at all, didnt try onstar
the ignition wire was always hot off the gmos04, as in it wasnt switching with the car
sound was absolutely horriable barely got loud and just tons of distortion

yes, I think I know what is wrong. Look on the package. Is there a date stamped on the front side? I believe you have one of the old interfaces, that should have all been sent back to the manufacturer.
